Dog Training & Walking Amendment
I,_________________________ (name of Pet Owner) HEREBY waive and release Claiborne Canine, LLC (name of Trainer) from liability pertaining to the matters set forth below. I understand that by signing this “Amendment” I expressly and willingly agree to assume complete responsibility for any risk of injury, personal property damage, pet loss/theft or death that may arise while “Trainer” is walking and/or training my pet. On behalf of myself, I waive all claims for damages, injuries, diseases, pet loss/theft and death sustained to my pet, that I may have against the “Trainer” relating to the pet walking and/or training. I understand that my pet could be lost, stolen, escape, be injured and die while with “Trainer”. By this waiver, I assume any risk and take full financial responsibility for damages to personal property, injuries, diseases and loss/ theft and death pertaining to my pet associated with “Trainer”, including, but not limited to cuts, or puncture wounds, scratches/chewing, accidental disease from outside or the “Trainer” themselves. If my pet is harmed during the duration of the pet walking and/or training, I will not hold “Trainer” responsible for any damages. I have informed “Trainer” of any physical limitations, ailments, or physical/mental disabilities my pet may have that would cause him or her to be uncomfortable, aggressive or contagious towards other animals or people.
“Trainer” agrees to perform the following specific duties:

Walk and exercise the pet according to the instructions and time specified in the House and Pet Sitting, Training and Walking Agreement.

Train the pet according the training plan.

Train the pet according to the training sessions agreed on by “Owner “and “Trainer”

Other
Any other services provided by “Trainer” not included in this contract shall be discussed by “Owner” and “Trainer”. The “Trainer” is NOT bound by any contract to provide services not included in this contract. The charge for any additional services provided will be agreed upon using text message. The charge for the additional service(s) shall be due on the last day of this agreement.
Cancellation and Rescheduling Policy for a scheduled training session is as follows: To provide consistent training and for other considerations you are allowed to cancel, postpone and reschedule a scheduled training session 1 (one) time. Any other cancellation(s) of a scheduled training session will simply be counted as a training session. Again, you may cancel a scheduled training session for any reason 1 (one) time. If there are any other cancellations of a scheduled training session then that cancelled training session will be considered fulfilled by the “Trainer”.
